In Trepang², players assume the role of Subject 106, an amnesic test subject who wakes up in a Horizon black-site. Initially handcuffed and relying on stealth, players must eventually fight their way through both human and paranormal enemies. The gameplay is reminiscent of mid-2000s first-person shooters.

During combat, players have the ability to slow down the action using a bullet time effect. They can also be aided by allies. The game features intense gunfights, gory visuals, and a trail of destruction. Trepang² offers a hardcore and frenetic shooting experience, allowing players to unleash all hell on their enemies while dodging bullets.
