First of all like the other difficulties, the enemies health is increased but that's not the problem while playing on that difficutly. All enemies, don't matter which one, are faster. Their movement speed got increased and even their attack speed. The bosses in this game have the same buffs expect Beneviento. The accuracy of the archers is close to 100% now. The amount of Lei the enemies loose now got quadrupled and sometimes you even get more. Some areas have new enemies in them but with new I mean tougher and more enemies than it was in the other difficulties. The damage you get is doubled compared to hardcore, I guess. But the biggest problem is that some enemies have higher knockback resistance. This includes all bosses and the normal enemies.
