Here's a playthrough of Super Smash Bros. Melee as I play all the unlockable stages along with all the unlockable characters.
According to Super Mario Wiki (http://www.mariowiki.com/Super_Smash_Bros._Melee) this is how to unlock each character and stage:

Jigglypuff: Clear Classic mode with any character or play 50 Melee matches.
Dr. Mario: Clear Classic mode with Mario without continuing or play 100 Melee matches.
Pichu: Complete Event #37--Legendary Pokémon or play 200 Melee matches.
Falco: Clear 100-Man Melee or play 300 Melee matches.
Marth: Use the fourteen default characters in either Classic mode or Melee matches or play 400 Melee matches.
Young Link: Clear Classic mode with 10 characters, including Link and Zelda/Sheik or play 500 Melee matches.
Ganondorf: Complete Event #29--Triforce Gathering or play 600 Melee matches.
Mewtwo: Play a combined total of 20 hours of Melee matches or play 700 Melee matches.
Luigi: Finish the first stage of Adventure mode with ##:#2:## on the timer or play 800 Melee matches.
Roy: Complete Classic Mode with Marth without continuing or play 900 Melee matches.
Mr. Game & Watch: Clear Classic, Adventure, or Target Smash with every other character or play 1000 Melee matches.
Superflat World: Flat Zone: Clear Classic, Adventure, or All-Star with Mr. Game & Watch.
Planet Zebes: Brinstar Depths: Play 50 Melee matches.
Eagleland: Fourside: Play 100 Melee matches.
F-Zero Grand Prix: Big Blue: Play 150 Melee matches.
Kanto Skies: Poké Floats: Play 200 Melee matches.
Mushroom Kingdom II: Obtain a Birdo trophy.
Special Stages: Battlefield: Clear All-Star for the first time.
Special Stages: Final Destination: Clear Event 51: The Showdown.
Past Stages: Dream Land: Clear Target Test with every character Past Stages: Yoshi's Island: Hit Sandbag at least 1,325 feet (400 meters) in Home-Run Contest.
Past Stages: Kongo Jungle: Clear 15-Minute Melee.
