Operation Potent Fog obscures game mechanics. Remember, this is the normal game with no mods! I haven't tested this yet, but I will soon... the sequence of events I performed which resulted in an accidental infinite-range grenade throw was:
25:40 move from no cover to cover,
25:45 clicking throw grenade right as the reload icon replaced it,
34:46 and then later reloading and throwing a grenade in rapid succession as the camera tried to zoom away on some random character.
Later, I'll try to iron out the details and see what are actual steps and what are just coincidental events.
