So this trophy is an utter bastard to get on unfair, so definitely try to do this on core difficulty, but it does have a fairly easy method. Arm every character with a ranged weapon, remembering that heavy crossbows are simple weapons anyone can use and then clear out the dungeon so you have access to the upper torture room with the body pit in the middle. Aggro the water elemental and in RTWP with nobody wearing any armour (it won't matter), lead it all the way to that room. Figure out who it's attacking, which will have a red line going from the elemental to that character and then play ring around the rosie in the center.

This is a shortened clip. It probably took about 30 minutes to do this but as long as you can jog in a circle it's 100% risk free and will eventually kill the little shit for your trophy. Make sure to keep the other ranged characters on standby so they don't accidentally get close enough to draw the elementals attention.
