Beat this on stream earlier today. Total time was 3 hours and 52 minutes, and it was a fairly hard clear. It wasn't hard. Literally per play through, from stage 1 to 5 (final boss) I'd get like 2 health drops that would drop. What made it hard wasn't the layout or enemy AI or anything, but the exceedingly rare health drops. Literally like 2 per play through makes the act of getting hit which is normally not a big deal in other games an extremely huge deal in that one. To beat it you're essentially running the entire game almost damage free, since there's limited continues, too.
My one scathing criticism about that game though is jump inputs get eaten. If you're laying on the ground (facing left for example) and stand up, and face right, there's some frames of animation where the character turns from left to right, and if you jump between those exact few frames it cancels jumps.
