A full Challenge run—this mode is an unfun, cluster**** of a mess! It is vital to play very methodically, gradually inching forward while eliminating as many enemies as possible. Attempting to rush through this will result in failure every time.
The Fire-Ice and Lightning potions will be your best tools here, along with charge shots to destroy off-screen monsters (abuse this!). Do not fall down into the lower world, or you're almost certainly finished, since all the enemies there will activate and begin bouncing everywhere, making for unplayable conditions.
After the very difficult second section, things become much easier until the end, with plenty of health items and checkpoints. You'll also earn a couple of extra lives by defeating the enemies. It gets tough again towards the end, however, so remain cautious.
