Step 1: Load up a Sonic 3 & Knuckles cart
Step 2: Go to any of the hook things in Angel Island Zone 1 and press these following buttons in order: Left (x3), Right (x3), Up (x3)
Step 3: After a ring sound is heard, pause the game and press A. After the title loads, press Up and select sound test. Go to Mushroom Hill Zone 1.
Step 4: On the other grabbing things, press the same button combos as in Step 2.
Step 5: After another ring sound is heard, pause and press A. Go back to sound test and select the same stage but this time press A+Start to enter debug mode.
Step 6: Go right and activate the Knuckles cutscene then backtrack to the beginning. Press B to activate debug mode, press A once, then rapidly go left and right placing the S monitors.
Step 7: Exit debug mode at the beginning of the row of monitors (by pressing B) and spindash into them. If your screen starts to get garbled, you successfully injected the code. On the garbled screen, press start and you are now playing Sonic & Knuckles. Enjoy!

TO FIX: Simply restart the console.
