Going completely fine for the most part(some concerning tendencies to walk in front of gunfire aside, though one person splitting off in the hall with the locked door might have been a warning)....

11:09 I call out where to go in the room ahead(right corridor), since it's a frequent point of failure.
11:22 I pings twice where to go in the room... (First ping was more a mistake, got the barrel. So pinged again to be clear. Note spam pinging isn't a thing or welcomed by anyone.)
11:30 V1TALCODEX ignores all this, and goes into the left corridor. (Both are fine, agreeing on one is preferable however.)
11:34 Party Horse also ignores all this, and is lingering by the entrance door while the backspawn comes up behind him.
11:44 V1TALCODEX and I both leave our chokepoints to try to help Party Horse, who's being overwhelmed and not coming to either of us.
11:53 Party Horse got broken out of the swarm, and starts to move towards V1TALCODEX and I
12:02 Party Horse was in the clear....and then dives back into the melee while I'm moving to join V1TALCODEX, who's taken up residence where I was originally.
12:08 Party Horse goes down in the middle of the room, V1TALCODEX and I both attempt a rescue...
12:11 I go down. (Tried to dodge, without stamina, wanted to use rockets/blastwave to cover Party Horse being picked up.)
12:24 Runners won't let V1TALCODEX pick me up, and he goes down too....and Party Horse has returned to the spot he got overwhelmed in again...where he gets killed by a Prowler.
12:39 Mission Failure
Not in video, but afterwards, both V1TALCODEX and Party Horse both saying I didn't do any callouts of where to go. :P
You end up with all three people doing completely different things and not supporting each other. The obvious happens.
Does it matter where we fought from? No, not really, everyone's a decent player here. Does it matter that we were isolated and not supporting each other? You betcha.
