To save Alex Mason from getting killed in the mission “suffer with me”, all you have to do is shoot him in the leg repeatedly which won’t kill him, and keep doing that until the cutscene plays and then, he will appear at the end of game, and it doesn’t matter which ending you get, he will still appear at the end of the game if you saved him.
