Black Ops 2 has a slight delay on aim down sights accuracy if you are moving and shooting. It's a relatively unknown game mechanic that is to blame for KSG inaccuracy issues and some of the more odd missed shots. I think it's a good thing as it mostly punishes run and gun users of LMGs and Assault Rifles, but it can be quit annoying on other classes. This video explains the reason it happens in BO2 and shows you how to help fix the problem with Quickdraw and Laser Sight.
