THIS IS HOW IT ALL GOT STARTED!! First Call of Duty from 2003, we start a VETERAN [Hardest] difficulty campaign.
Veteran setting, not only do you take the most damage of any of the settings (two or three bullets is usually enough to finish you off), all of the health packs and health kits are removed from the game, meaning that you are forced to play through each level with only a single health bar.
