This is Castlevania II: Simon's Quest

Often ridiculed and considered a bad game, Castlevania II: Simon's Quest was the first Castlevania game I played and thus set the golden standard more or less for me regarding Castlevania. While indeed confusing, I admire it's open world concept. My main complaint is just that you lose all the 'hearts' you've collected if you game over which can be utterly frustrating.

Taking place seven years after Castlevania where Dracula cursed Simon at the conclusion. A mysterious woman tells Simon that he must collect pieces of Dracula and burn them in order to lift the curse, or else, Dracula will be revived once Simon succumbs to the illness that the curse has wrought upon him.

Also famous for being the first game to have the song 'Bloody Tears'
My favorite Castlevania song ever.
