Edited: In a (failed) Yama Crysknife run, I experienced a strange glitch I'd never seen before where the ghost never spawned. It seemed to have been triggered by the fact that I was dying and being resurrected when the ghost would normally spawn.

I did a cursory check of the internets to see if people knew about this and didn't find anything, but after posting this on the Spelunky subreddit, they did indeed already know about this and actually had an explanation for it.

Since I died by falling into the abyss, the Ghost spawns at the same vertical height as you, which causes it to also be considered falling into the abyss, which is always instant death.
