Dark Souls 2, No-Death Run (NG), Part 2

This segment is where I get all four Great Souls and open the Shrine of Winter. I'm mostly done collecting (though I do get my Mace up to +10) and just want to work through levels and kill bosses to progress with the game. The goal is to have a safe, repeatable path through each zone, but overall to try to keep things moving reasonably quickly. The longer the run gets, the more risks there are, and the more painful it would be to have to restart.
