UPDATE: The punched card can also be found in the inventory of ZmbM_usSoldier_Officer_Convoy , an infected that spawns with the new Dynamic Convoy events. Phew!!!
As far as I'm aware, as of DayZ Experimental Update 1.19, there is only one underground bunker, and it's near the South-West Corner of Livonia, near Dambog, at the approx coordinates of "735.242371, 1228.524414 ".
You will need the punched paper access card, which can only be found in, and spawns at, the Contaminated Zones / Areas at the Glinska Airfield (4085.47 / 10301.41) & at Radunin (7447.62 / 6191.62).
