Thorny Towers is one of those areas that isn't necessarily disturbing at first glance, but which gets worse and worse the more you think about it, the more you take it in and immerse yourself in it. Sure, the remaining inmates are cordial enough, but the state of the building is in shambles, it's dark, it's dreary, it's in ruins, the upper floors are twisted in ways approaching non-euclidean, there are screaming and crying faces of agony in the clouds...

And the music, while not initially sticking out as particularly frightening, has a lot of little touches that make it uncomfortable to listen to. People usually point out the high pitched squealing sound around thirty seconds in as the chief offender, but what always disturbed me far more was the sound that your hear at about 1:06, right as the backing starts to crescendo back into the loop.

It sounds like... I don't know. The closest thing I can approximate it to is the sound of someone letting out a whimper or a yelp from receiving a blow to the stomach or chest... underwater... or smothered in something. Not a great thing to think about when you're in an insane asylum.
