Doom (1993) using GZDoom source version
"Ultra-Ultra Violence" Difficulty using the -fast command to increase the enemies speed to Nightmare difficulty without them randomly re-spawning and no double ammo for me.
Activating this mode is easy with GZdoom; simply go to the options menu, go to game play options and the Fast Monsters option is half way down the list.
Episode 1 "Knee Deep in the Dead"
Mission 9 "Military Base"

This is a secret level that is accessed from Mission 3 "Toxin Refinery"; when you complete this level you will be brought to the start of Mission 4 "Command Control" as normal.
