Ultra-Nightmare difficulty is unlocked after you finish the game. To get this achieve Ultra-Nightmare in new slot. That means no upgrades, no runs, no mods.
On this difficulty enemies hit a lot harder and every charged attack will kill you, so constantly move and avoid splash damage. DON'T RUSH, don't charge in to the fight. Stay behind and wait for enemies to come to you.
Also get all secret to upgrade your weapons, you can skip armor upgrade as barrel explosions will cripple you at best.
