This is Annihilate Me difficulty which is default for coop and has more monsters than Ultra Violence.
Note: This level can get bricked at the end in coop. Naughty level designers! All it would have taken is adding a portal to teleport up to a major fight area once it starts, in case you die and get locked out of there by the monsters.
