"Rescued 5 other players in co-op play."
Enter a co-op mission (online or local co-op); you need to play with real players.
During the mission, wait for a teammate to get “downed” (i.e. knocked out / incapacitated).
Move to their location and perform the revive / rescue action (often by pressing the button prompted on screen) to get them back up.
Repeat this rescue action until you have rescued 5 different instances of other players. They can be the same or different players, as long as there are 5 rescues total.
After the 5th rescue, upon mission completion (or immediately after the 5th rescue in some cases), the trophy should pop.
