Ran into a bug while playing today where no enemies or npcs would spawn in the world.
I also couldn't summon my horse or use sites of grace.
Think it was triggered in another one of my videos while visiting merchant at the academy gate.

After talking to him, I couldn't ride my horse.
I teleported back to the academy gate waypoint after that.
Next I went through the sealed area to the other side and noticed there was no finger reader and I couldn't use sites of grace.
I explored for a bit a noticed some weird interactions like saurons eye not causing madness and boss battles being bugged.
Game crashed when trying to quit, but relaunching game fixed issue.
