Be at Ground Zero of a Nuclear Blast
For this trophy, you have to be inside the blast zone of a nuke the moment it hits.
When adventuring in Appalachia, a side quest called Death From Above! might start. This means someone on the server launched a nuke, and a countdown of 3 minutes starts until the explosion. Starting the quest will mark a red circle on your map, you need to get inside the circle before the nuke hits. You can't fast travel into blast zones, so you'll need to fast travel to a nearby location and walk inside the circle.
When the nuke hits, the blast will instantly kill you (provided you're inside the blast zone) regardless of your protection. When you die, the trophy will pop.
