I believe this occurred when the poison gas (spawns upon a termination) or perhaps the burning effect, killed an enemy exactly at its spawning moment, so the enemy loaded, but with no hit box or HP.
Letting the folks at @gearboxofficial and @MagicDesignStudios have a look.
Didn't get the moment the glitch occurred, sadly, but this is the Natural Disaster Department. Other events were the holiday dialogue, and the facilities tour was also happening.
