Here we have the heart transplant level on surgineer quest 2 VR.

This was one that was difficult to do since you are not told what the tools are or what they do.

but we've figured it out!
you need to cut open the chest, smash the rib cage with either the hammer or axe,
remove the lungs from the chest and then remove the heart.
replace the heart with a new one and replace the lungs, install the new rib cage and place the chest skin back on.
Once you've done this. shock your patient and that is level complete!
