STORY: Hitman: Blood Money ended with Diana helping 47 to fake his death, in order for him to escape The Franchise. 47 killed their leader, former FBI director Alexander Leland Cayne, though he was part of a larger secret society called Alpha Zerox. Diana and the rest of ICA do not know 47's whereabouts, nor does the player for certain - though the last scene shows 47 entering an Asian (possibly Chinese) establishment, asking suspiciously what they have to offer, "preferably in the back". The first in-depth trailer for the game shows Agent 47 storming a safe-house, and confronting Diana while she is showering. 47 is "betrayed by those he once trusted and now hunted by the police, he suddenly finds himself at the center of a dark conspiracy and must embark on a personal journey through a corrupt and twisted world, in his search for the truth". At least one portion of the game will be set in Chicago, but it has been confirmed that the rest of the game will take place within the US.
In the first mission, 47 executes a mission to kill Diana in her mansion under the order of the Agency as Diana found incriminating evidence about the ICA. Diana then asks 47 for a final request, which is to find and protect a girl called Victoria. As 47 accepts Diana's dying wish, the ICA deem 47 a traitor.
47 drops Victoria off at a Catholic orphanage and goes to see an Agency informant called Birdie. Birdie offers information but only if 47 will kill "The King of Chinatown." 47 does so and Birdie directs 47 to a hotel called Terminus where a man named Blake Dexter is conspiring to kidnap Victoria, hold her for ransom, and make the Agency pay to get her back. After cutting the barcode tattoo out of his head and giving his Silverballers to Birdie, 47 infiltrates the hotel, and climbs the stairs to Dexter's penthouse. When 47 attempts to take out Dexter's guard Sanchez, he is knocked out and, when Dexter realizes who he is, framed for the murder of a cleaning lady of the hotel, whom Dexter killed. Dexter sets the room on fire and leaves 47 to die. Once he comes to, the police are attempting to get into the hotel room. He escapes the room, scales the ladder outside the window, and runs along the rooftops when finally crashing into a library (the Run For Your Life gameplay trailer showed this). Eventually escaping, 47 calls Birdie and Birdie turns him onto an associate of Dexter's named Dom, who runs a strip club. 47 kills Dom, evades the police again, and heads to Chinatown. During the Chinese New Year, three henchmen of an assassin named Wade, who is an employee of Dexter's, come to kill Birdie and figure out where 47 is. 47 kills the three henchmen and upon discovering that Birdie has disappeared, 47 heads back to the orphanage to check on Victoria. Discovering that the orphanage is being attacked by Dexter's men, 47 attempts to get Victoria out, but she is captured by Wade and his son Lenny.
Meeting up with Wade and killing him, 47 heads to South Dakota to take out Lenny and Dexter. After getting his Silverballers back, 47 takes out Lenny's gang, interrogates Lenny about Dexter and shoots Lenny in the middle of the desert. Taking out Sanchez in a wrestling match, 47 heads to Dexter's headquarters and while he is there, kills his entire research and development head staff and figures out where Victoria is. 47 dons a new suit and heads to the pickup. He kills Jade and finally Dexter. He retrieves Victoria and leaves the helipad in which Dexter was killed.
The game goes forward a few months and the head of the Agency is attempting to dig up Diana's remains. 47 ambushes the cemetery and kills the top sniper assassins of the Agency. In Diana's mausoleum, 47 blows the door and the head of the Agency gives up and asks 47 if he really actually killed Diana. 47 takes the silencer off of his Silverballer and, with a furious look in his eyes, replies "You will never know" as he shoots him in the head.
The ending cutscene shows Victoria standing on the balcony of the Burnwood Mansion. A woman walks up behind her, revealed to be Diana. As 47 watches from a sniper scope, Diana says "Welcome back, 47."
Agent 47 packs his rifle away into the suitcase and walks away, presumably to the next hit.
The final cutscene shows Birdie walking into the chief of police's office, who has set up a tree graph to figure out who 47 is. Birdie says he will help the chief find 47....for a price.
