With the Nabazov Virus safely contained in Colorado, 47 must now end the threat of the virus for good by eliminating the man responsible for its creation, rogue viral researcher Owen Cage.

Intending to become the virus' 'Patient Zero', Cage deliberately infected himself with the virus, and was attempting to spread it in Sydney, Australia. Luckily, before he could travel he was detained by authorities and sent to be quarantined at the GAMA hospital in Hokkaido. There, researchers from the Ether Biotech Corporation have been able to create a vaccine. But the lead researcher, Klaus Liebleid, has been keeping Cage alive for study, likely attempting to reverse engineer the virus for Ether's use.

47 must infiltrate the hospital and eliminate Cage and Liebleid, as well as any people within the building infected with the virus. All while avoiding becoming infected himself.

Despite being isolated, the Nabazov virus will begin spreading from Owen Cage to other people present in the mission if the player does not intervene. The virus can potentially spread to every NPC within the mission area; anyone not wearing a Bio Suit can be infected given they come in close enough contact to an infected NPC. All those infected with the virus become additional targets.

47 can also become infected with the virus. If the player makes direct contact with Owen Cage or another infected NPC without wearing either the Bio Suit or Head Researcher disguise, 47 will become infected and the player must locate and use the virus' vaccine before leaving the mission. If five minutes pass before the vaccine is used, 47 will die and the mission will fail.
