Please Read:
First of all you need PES 2018 Game.
You have to install Pes 2018 & the patches on the same external drive (Either external HDD or Flashdisk).

Your external HDD or Flashdisk should be formatted on xbox 360 so it will be detected by Horizon. The patches need 20GB storage cache. Setting it at least 21GB is recommended. The less the size of the drive, the game would run faster.

The patch is not perfect. Some players are still in their former club but it's better than nothing. You have to move players manually.
