A guide on completing the main quest line in The Elder Scrolls IV: Oblivion on the Highest difficulty setting at level 1.

This means no extra health, stamina or magicka from level ups and enemies will do 6x normal damage to the player and the player dishes out 1/6th normal damage to enemies .
Complete with the most up to date version of Oblivion with no mods installed.

Due to Oblivion's levelling mechanics, Levelling skills not taken as a major skill at the start of the game will not raise you in level. Enemies scale with your level so keeping the game locked at level 1 is an effective way to play on the highest difficulty setting.
Summons and followers are not affected by the difficulty setting, they will do the same damage on the lowest setting as they do on the highest.
