Unlike most Big Buck Hunter games, In Case Of Zombies has a bit of a story line to it. Pappy has created a new recipe for his moonshine using toxic waste. This causes anyone that drinks it to turn into zombies. He calls in Zombie Exterminator Ed Brodeen to clear his home of the flesh-eating creatures. Regular zombie humans and deer can be headshot for quicker kills and bonus points, zombie chickens and raccoons attack in packs that must be shot quickly, and the zombie boyfriend and bear can be stunned/dismembered by shooting the targets on their bodies. Glowing moonshine jugs can be shot for extra points. When a player's health runs out, they have 30 seconds to continue (if paying with a credit/debit or player/cash card, it changes to 74 seconds).

The gameplay feels like your typical arcade light gun shooter, where you are armed with a shotgun and shoot zombies that try to eat you. Other than standard zombies, there are also zombie animals as well. However, unlike most shooters, In Case Of Zombies has only 3 levels and can be completed in about 10 minutes. Most modern arcade shooters tend to last around 30-45 minutes, if the player's goal is to complete them.
