You most likely have to get the "Best" ending for the achievement, so that's what I did.
This achievement can be acquired on any difficulty.
"True" is the hardest difficulty in the game. "Masochist", from the previous game, has been removed. The new "Throwback" difficulty is actually easier in most ways due to how it's meant to be played.
