Random reset glitch, shown twice in this video. From what I've gathered, this glitch has been around ever since the Cold Stream DLC was released for the console version. The game just blacks out and then resets. I have so far only had this happen whilst playing in single player mode.

This has so far only happened to me on The Parish campaign, but others have reported it also happening on Dark Carnival. There have been a few suggestions on how to fix it but the only one that worked for me is the worst one: delete the Cold Stream DLC from your console's hard drive. Fix this please Valve!
