2During a siege, the river is a rather dangerous way to get fresh water. The lake in the cavern is a way to mitigate that. Simply drill a hole above and you have yourself a steady water supply. Provided you also have a sufficiently long rope. We also set up a hospital area, so wounded dwarves can receive medical treatment (assuming the doctor is not drinking).
