Endgame. The final stage is long and has some seriously mean enemy placement, let alone painful stage hazards. The first phase of the final boss can be hard to dodge, but thankfully the game gives you an ideal subweapon for it. Phase 2 is harder to dodge than f=phase 1, but easier to learn the pattern of. I have no idea how Phase 3 is a thing, but it's nothing too bad compared to the final bosses of Ninja Gaiden 1 or 2 with the version of the game being played here.
I'm playing the MottZilla "Ninja Gaiden III: Restored" hack which brings the game back to it's Japanese difficulty setting, as the US version jacked it up for rental stores. You can download the patch for the game here: https://www.romhacking.net/hacks/570/
