Remember the part of the game where the world unceremoniously went belly-up and we were tossed in a would-be Tokyo with no clue as to what the hell happened? This is how you learn about the Vortex World, Kagutsuchi, the Conception, everything. This is how you get the actual plot of the game, by fighting generally very powerful optional bosses and going through HELL ITSELF. Some games give you unskippable cutscenes just for walking within 200 feet of the villain of the day, this game makes you go through Hell just to see them.

Also in answer to the lady in black's question, worlds are born, destroyed and reborn so as to perpetuate existence as human and demon knows it. Be born so that you may discover the world and struggle to find a place while fighting off those that would take it from you, eventually die when that struggle died down and ends, be reborn to start the conflict anew. Without that cycle existence would quickly become stale and become little but a glorified space version of what would otherwise be an infinite void devoid of anything and everything. Apparently reality falling into that void is against the Great Will, be that Great Will Jehova/YHVH or Brahman, or some other suitably powerful entity.

...I think, the purpose of existence and how the concepts of life and death affect it are a little tricky to work with, as are the respective creators of various levels of reality from about a dozen mythologies. I can say that if the above it true, it raises questions as to why Hikawa doesn't have every angel in the Vortex world gunning for him; his world of Stillness would erase conflict if it came into being.
