Of all your squadmates from the Mass Effect Trilogy, Miranda is one of the most likely to die in Mass Effect 3. If you make the wrong choices during the course of the game, then Miranda will die when Shepard and crew infiltrate the Cerberus compound on Horizon. She will either be killed by Kai Leng, who attacks before you arrive, or by her father during the standoff.
There are three requirements to save Miranda in Mass Effect 3:
1) You must complete all three conversations with her on The Citadel prior to the Horizon mission
2) You must inform her that Kai Leng is still alive after Cerberus attacks the Citadel
3) You must choose the correct options during the standoff with Henry Lawson on Horizon
If Shepard was in a romance with Miranda in Mass Effect 2, there is an additional requirement: you cannot break up with her in Mass Effect 3. If you dump Miranda in ME3 then she will die no matter what you do.
