1993年12月17日に発売されたスーパーファミコン専用(SFC)のアクションゲームで初の登場。

[ストーリー]
初代『ロックマン』から100年後[注釈 1]。ライト博士が研究中未完成のために数十年前に封印したロボット、エックスが主人公の物語。
西暦21XX年[3]。人間と人間に近い思考回路を持つロボット「レプリロイド」が共存する世界。人間が犯罪を犯すのと同じく、レプリロイドや、人間に使われるだけのロボットであるメカニロイドでさえも犯罪を犯す存在、「イレギュラー」が現れ始めていた。それを速やかに解決、抹消する特殊機関が「イレギュラーハンター」である。ケイン博士によって封印から目覚めたエックスは、B級ハンターとしてこの機関に所属することになる。
ある日、謎の赤いイレギュラーが暴走しているとの知らせを受け、シグマ率いる第17精鋭部隊が出撃した。しかし、その圧倒的な力に並みのハンターではかなわず、隊長であるシグマが戦いを挑む。ついに、シグマはそのイレギュラーの鎮圧に成功したが、その日からシグマの様子はおかしくなっていったという。シグマによって鎮圧された赤いイレギュラーは「ゼロ」と呼ばれ、その後は正常な機能を維持。シグマが監視を兼ねる意味でイレギュラーハンター第17精鋭部隊に配属となった。そしてエックスとゼロは先輩後輩の仲から親友となり、ゼロは特A級ハンターとなる。
それからしばらくした頃、シグマが突然人間に対して反乱を起こした。その計画は人類を排除し、レプリロイドだけの世界を築こうというのである。それに乗じてイレギュラーハンターの隊員もシグマ側に付く者もいた。残されたハンター達は戦いを挑んだが、シグマ勢の圧倒的な力に次々と倒されてしまう。
そしてエックスとゼロはシグマの計画を打ち砕くため、たった2人でこの大規模な反乱に立ち向かうのだった。
[Story]
100 years after the original "Mega Man" [note 1]. The story is about X, a robot that Dr. Light sealed away decades ago because his research was incomplete.
The year is 21XX [3]. A world where humans and robots with human-like thought processes, called "reploids," coexist. Just as humans commit crimes, "irregulars," which are even among Reploids and Mechaniroids, robots used only by humans, have begun to appear. The "irregular hunters" are a special organization that quickly solves and eliminates these. X, who was awakened from his seal by Dr. Cain, becomes a member of this organization as a B-class hunter.
One day, upon receiving news that a mysterious red irregular was running wild, the 17th elite unit led by Sigma was dispatched. However, its overwhelming power was no match for ordinary hunters, so Sigma, the captain of the unit, challenges it to a fight. Finally, Sigma succeeds in suppressing the irregular, but from that day on, Sigma's behavior began to become strange. The red irregulars that Sigma subdued were called "Zeros" and continued to function normally afterwards. Sigma was assigned to the 17th elite Irregular Hunters unit to supervise them. X and Zero went from being senior and junior to becoming best friends, and Zero became a special A-class hunter.
A little while later, Sigma suddenly started a rebellion against humans. His plan was to eliminate humanity and build a world of only Reploids. Some members of the Irregular Hunters took advantage of this and sided with Sigma. The remaining hunters tried to fight, but were defeated one by one by the overwhelming power of Sigma's forces.
So X and Zero faced this large-scale rebellion alone to thwart Sigma's plan.
