Cruz Silva was the daughter of former soldier Gregorio and grew up as a childhood friend of Louis Amamiya.
During the crisis of the Lost plaguing the land and the frenzy-enhancing Miasma they unleashed, Gregorio established Project QUEEN, an attempt for the Revenants to reduce their bloodthirst. Cruz willingly allowed herself to be used as a test subject to become the "Queen", where she was exposed to the blood of every organism the project could obtain. The project was an initial success and the Queen did not experience any thirst for blood. With her new powers, the Queen created multiple materials to aid the Revenants. One of her inventions was Mistle, a plant made using medicine from the Queen's blood that not only regenerated the Revenants' wounds and quenched their thirst, but also slowly degraded the Miasma in the air.
Able to withstand the unimaginable pain from the experiments she chose to endure in order to save mankind, partly due to her caretaker Karen and her close friend Louis, she then became a powerful incarnate who even brought destruction on top of the Great Collapse. Despite her frenzy, she has somehow lost her senses, but still tries to reach out hoping someone would hear her plea.
Although she is described as "immortal among immortals", the Protagonist is the one to "kill her" by impaling her with their blood veil. As the Queen's blood is injected into the Protagonist's blood, they gain her Blood Code and some of her abilities.
The parasite within the Queen took a toll on her powers and caused her immense pain. Consumed by her agony, the Queen frenzied and unleashed a massive quantity of Miasma. With the new rise of Lost and the Queen backing them up, the surviving population was quickly decimated. Gregorio initiated Operation Queenslayer with the intent of killing the Queen. The Queen herself created an army of highly powerful Lost called the Queen's Knights to protect her. In the battle, the Queen would finally be slain by the player character who destroyed her heart but was exposed to the Queen's blood. Jack attempted to kill the player before they frenzied, but they lived as he failed to destroy their heart.
The Queen's soul was separated and her individual powers were sealed away within special Visages called Relics to prevent her from resurrecting. Those given the Relics became Successors, gifted with the individual power of the Queen, but this drove them to madness and became extremely powerful Lost. The player's in-game goal is to collect all the Relics so the Queen could not resurrect.
