Hell of rambly video here, my apologies for that. It's impossible for me to keep my thoughts on the one point because all of this stuff interrelates and real world weight loss is not neat, tidy or linear. There are no simple steps to follow that guarantee results and account for all the variables with each and every one of us, you have to dig deep and figure out what the physical, but mainly emotional and mental issues are in your life that are causing your endocrine system to stay out of whack. Once you deal with them and get your hormones in line you will start losing weight, and it will be effortless and permanent, as opposed to a constant battle against your body that you're doomed to lose given enough time because the body holds every single card for controlling how much weight you gain or lose.

If you're interested in reading deeper on this subject I highly recommend looking into the work of Jon Gabriel, insulin and leptin resistance, the Vermont Prison study (good luck finding the original!) and the set point theory. That should get your started.
