Playing with loons(companions) on Unfair is a challenge on it's own(Extremely hard from start to finish), playing solo will give you more xp(making the game easier in the first 200h and harder in the last 100h) while playing with mercs(if you not what you're doing) is Unfair mode but for enemies.
Loons are just bad, from the start, with their wasted points in feats and there is nothing you can do about it since full respec is not available on both hard and Unfair difficulties.
There are encounters that are missing from this list, but they will get changed with patches for sure and their difficulty depends on your choices too. These 10 encounters remain the same no matter what choices you decide to take. There are a lot of difficult fights that just did not make on the list like Areelu Vorlesh(Fight depends on choices you took for how hard it will be), all Threshold encounters(broken, cause no rest), Playful Darkness(you can always come back later to fight him when you get few levels and he becomes easy), Pathetic Quasit(You can burst him in matter of seconds if positioned well), Hosilla(Nerfed hard since vanila version), and many many more...
