Ruthless - formerly codenamed 'Hard Mode' - is coming in 3.20. Like the long-established SSF ruleset, it's an opt-in difficulty setting that people can choose to play, but won't affect you if you don't play it.
The short version: I've had a lot of fun, but this mode isn't going to be fun for everyone.
