The final map is quite difficult on Nightmare, as there is no armor to be fetched anywhere, at least I didn't find one, so you gotta do with 50 HP. So you have to traverse through an underground pavement full of Shamblers, Vores and various spike/nail traps. The Quad Damage at the end is tough to get earlier on, so it can't be used against majority of enemies. The final boss don't really do anything, the goal is to telefrag her when the floating spike thingy touches her, as you teleport where the floating spike thingy is and that's it.
