As in the film, the plot of the game revolves around Vietnam War veteran John Rambo, who is called back into service one last time to rescue his former commander, Colonel Samuel Trautman, who was kidnapped during a mission in Afghanistan.

Players can play as either Rambo or Colonel Samuel Trautman, each armed with a machine gun and explosive arrows.
The goal is simply to reach the end of the level by shooting everything that moves between enemy soldiers, helicopters and tanks. Both characters have a health bar, depleted of which the game ends.
Both players are always running, shooting enemies, and collecting various objects including power-up and healing items.

The game has 5 scenes / stages inspired from the movie with different enemies formation and sometimes even unique scrolling backgrounds.

The support co-op local multiplayer.
