In 1899, after a botched ferry heist, Dutch van der Linde and his gang – which includes protagonist Arthur Morgan and a young John Marston – are forced to flee their hideout in Blackwater and attempt to traverse the United States east to evade law enforcement. To fund their escape, Dutch has the gang rob a train belonging to the wealthy oil magnate Leviticus Cornwall. A furious and vengeful Cornwall then hires the Pinkerton Detective Agency, headed by Agents Milton and Ross, to apprehend the gang. As a result, the gang is forced to remain nomadic to avoid law enforcement. All the while, Arthur and the rest of the gang perform numerous jobs and heists to keep the gang funded as Dutch continually promises one final heist that will achieve financial security and eventually freedom.
After shooting up the farming town of Valentine, New Hanover, in a pyrrhic bank heist, the gang moves on from their initial camp near the town to a new one, west of the small rural town of Rhodes in the southern state of Lemoyne. Within Rhodes, the gang learns of old Yankee gold which belongs to one of two warring families, the Grays or the Braithwaites, who gained their fortune off the slave trade. Initially working for both sides in an attempt to play them against each other for the gold, the gang's duplicitous actions are discovered by both families, culminating in a battle against the Gray family at Rhodes in which gang member Sean is killed. A battle at the Braithwaite manor ensues, where the head matriarch reveals she has kidnapped Jack, John's son, and given him to Italian crime lord Angelo Bronte, who resides in the city of Saint Denis. The gang is tasked by Bronte to commit illegal acts on his behalf, providing Dutch a just reason to attempt to rob Bronte and the wealthy elite of Saint Denis. This leads to a poorly planned bank heist that results in the deaths of both Hosea, a veteran member and best friend to Dutch, and Lenny, a young African-American member who was eager to prove himself. John is apprehended by the Pinkertons and later incarcerated; Arthur and Dutch, along with fellow van der Linde gang members Bill Williamson, Javier Escuella and Micah Bell, escape via a tanker ship on a voyage to Cuba.
A torrential storm causes the tanker to sink, and the men becomes lost at sea. The gang is stranded on a sugar plantation island named Guarma, where they become embroiled in a civil war between the plantation owners and the marginalised workers forced into life on plantations. While on Guarma, Dutch's actions gradually become more extreme and violent; Arthur becomes disillusioned when Dutch strangles an elderly female guide to death out of fear she would turn the gang in to the local authorities. They succeed in furthering the revolution, and eventually return to the United States.
After their arrival, the group continues to run into trouble and strife, largely due to Dutch's continual descent into violence and risky behavior. Arthur and fellow gang member Sadie Adler rescue John from prison, but he begins growing increasingly disillusioned with Dutch's leadership as he becomes more paranoid. Several members of the gang bolster Dutch's decisions into escalating violence, including the ambitious and unscrupulous Micah, who advocates more suspicion of the group. In addition, Arthur discovers that he has contracted tuberculosis following an earlier altercation with diseased farmer Thomas Downes, giving him only a short time to live. This causes him to reflect on his life and his negative actions, including his loyalty to the ever-deteriorating ethos of Dutch, which included manipulating a local Native American tribe into a conflict with the United States Army to buy the gang time. Several gang members become uncomfortable with Dutch's repeated failures, broken promises and overall downward direction of the gang, and quietly leave. Molly, Dutch's love interest, is killed by camp matriarch Susan Grimshaw after admitting she had been keeping in contact with the Pinkertons, constantly informing the agency the gang's location. Following yet another train robbery, Arthur convinces John to leave and start a new life with his family. Arthur's loyalty to Dutch breaks when Dutch murders Cornwall, leaves John to die, and refuses to rescue John's non-statutory wife Abigail when she is captured by Agent Milton. Arthur disobeys Dutch's orders and rescues Abigail, killing Agent Milton and learning that Micah has been working as an informant for the Pinkertons.
