Overview
The many years of playing SegaSonic The Hedgehog, there was only the idea of thinking that there was the Japanese version that exists, however I recently came across an article that shows that there is a translated version, featuring a different version of Robotnik!
As mentioned in the Article:
The most intriguing and puzzling aspect of the game is the fact that within its data is a complete English translation of the (admittedly few) lines of dialogue and a full makeover of Eggman, turning him into Robotnik of the "SatAM" cartoon series that was airing on TV around the same time of the game's development. Neither the Robotnik graphics nore the English translation are implemented in the code and go entirely unused.
It's not that this localization was started then abandoned half way: it is complete in practically every respect. And there's evidence for both a World region version as well as a US version. But as far as anyone knows, it has never had a release outside of Japan.
Whether there is or is not an undumped version out there, the final version has everything needed to reconstruct an English version with a pretty high degree of certainty. So the creators that made the article did just that. You'll find in the link down below two MAME cheats which will show the text in English and convert Eggman into Dr. Ivo Robotnik.

To my understanding, these codes only work on the arcade emulator, MAME. This was showcased using the latest version, which is version 0.267.
Wish there was a way to change the difficulty, but at the same time I'm not sure if changing the difficulty would make any difference throughout the gameplay from what I've seen previously from a couple of playthroughs that exist out there.
