Notable because it's pretty rare for me to die in a Warriors game, usually if you fail it's because an NPC ally got swarmed.

So I found out that enemy Shamir can freeze you with her normals, I wasn't expecting her to still have her player-controlled abilities. I was mashing buttons but nothing got through. Maybe I could have switched to a different team, but it took only 7 seconds for Hubert to go from half-dead to completely dead.

Definitely should keep an eye on Shamir while she's attacking, and use Warrior Special to close things out safely if you have it.
