You must be big Mario to reach the secret exit and fortunately there is a Mushroom at the start of the level.
Drop down onto the yellow blocks and go all of the way to the right. Spin jump through the first group of yellow blocks and continue through the 2nd group, still along the right wall, and you'll eventually reach the key and keyhole.
