Inspirado en la historia bíblica de Isaac,1 el protagonista y su madre viven felizmente en una pequeña casa en una colina, con Isaac dibujando y jugando con sus juguetes, a la vez que su madre ve transmisiones cristianas en la televisión. La madre de Isaac escucha entonces “una voz superior” que afirma la corrupción de su hijo por el pecado, necesitando una salvación: quitarle todo lo que está mal. Por ello su madre le quita sus juguetes, fotos, consola de juego e incluso sus ropas en un intento de salvarle. La voz habla una vez más a la madre, instándola a separar a su hijo de todo lo que está mal en el mundo, con lo que encierra a Isaac en su habitación, hasta que la voz habla una última vez, cuestionando la devoción de la madre y pidiéndole el sacrificio de su hijo. Ésta agarra un cuchillo de cocina, y cuando camina a la habitación de Isaac, el protagonista entra en pánico, da vueltas por la habitación y encuentra una trampilla bajo la alfombra en la que salta al sótano. Allí se encontrará con hordas de enemigos perturbadores y desquiciados, hermanos y hermanas perdidos, miedos, y finalmente a su madre. En un final el jugador puede decidir en tomar el cofre al derrotar a Mamá, o seguir adelante y derrotar a Satanás

En los puntos de carga del juego, se muestran los personajes acurrucados en una bola, llorando, excepto "???", que se muestra muerto. Visibles son sus pensamientos, que van entre el rechazo de su madre, la humillación de sus compañeros, y el escenario de su propia muerte. El juego cuenta con múltiples finales.

Inspired by the biblical story of Isaac, one the protagonist and his mother live happily in a small house on a hill, with Isaac drawing and playing with toys while her mother watches Christian television broadcasts. Isaac's mother then listen "superior voice" says the corruption of his son for sin, needing salvation take away everything that is wrong. In the final the player can decide to take the chest after defeating Mom, or go ahead and defeat Satan
