Filling out the Entry Form on our webpage means you agree to the following terms:

1. To qualify, you must subscribe to our YouTube Channel – 5 Minute Reviews.
2. Pre-existing subscribers are eligible to participate, just fill out the entry form.
3. Your YouTube account must be at least 6 months old.
4. You are eligible for additional entries per person you convince to subscribe to our channel – simply have them note your YouTube account name in the “additional comments” section of the entry form. HOWEVER, we will only accept additional entries if the YouTube account of the individual you convince to subscribe is at least 6 months old.
5. Unsubscribing from 5 Minute Reviews will automatically disqualify your entry. In addition, if your friend unsubscribes, your additional entries will be made void.
6. Upon winning, you will be contacted via email to arrange the prize delivery. If you do not respond within 3 days, or if we are unable to get in contact with you, the prize will be awarded to the runner up.
7. If you win, both the prize costs and shipment fees will be covered by 5 Minute Reviews.
8. We will not share participant personal information with any third parties. Any information gathered from participants will be to:
a.) Keep track of contest participants
b.) Contact winner(s)
c.) Ship prizes to winner(s)
Upon completing the contest, meaning the prize has reached its destination, we will dispose of all personal information collected from participants.
9. Entries which do not comply with YouTube’s Community Guidelines will be disqualified.
