La trama central de la expansión Cataclysm es el regreso del aspecto del dragón maligno Alamuerte el Destructor (originalmente Neltharion, el Guardián de la Tierra). Visto por última vez en Warcraft II, que tuvo lugar hace más de dos décadas, Alamuerte ha pasado ese tiempo curándose a sí mismo y planeando su ardiente regreso desde el plano elemental de Deepholm. Su regreso atraviesa la barrera dimensional dentro de Azeroth, causando un cataclismo arrollador que remodela gran parte de la superficie del mundo. En medio del desastre mundial viene el conflicto renovado entre la Alianza y la Horda, que ahora está bajo el gobierno de Garrosh Hellscream. Con los reinos elementales ahora abiertos al mundo, los espíritus caóticos elementales y sus señores tiránicos surgieron para ayudar al Destructor y al culto nihilista del Martillo Crepuscular para lograr la Hora del Crepúsculo: el fin de toda la vida en Azeroth.
